How Much LegitScript Certification Costs a Treatment Center

LegitScript charges an application fee and an annual fee per facility, plus extras. The real first-year cost for a single-site center and what it unlocks.

If you run an addiction treatment center in the US or Canada and want to advertise on Google, you need LegitScript certification first, and it isn't cheap. The LegitScript certification cost for a single facility is a nonrefundable application fee of $1,395 to $1,595 plus an annual fee of $2,550 to $3,095, so roughly $3,945 to $4,690 in the first year before you've spent a cent on ads.

That's the published price. The real cost includes the work to meet LegitScript's standards, the lead-generation relationships you may have to end, and the chance that certification still doesn't get your ads seen. Here's the full budget, what certification actually unlocks, and how I'd decide whether it pays back for your center.

What LegitScript charges

These are the fees on LegitScript's addiction treatment certification page. The range depends on your situation, so check the current figures for your program before you budget.

Read that last row carefully. Paying to expedite gets your review started quickly. It doesn't promise a faster decision or a positive one.

The real first-year LegitScript certification cost

For a single-site center, the arithmetic looks like this:

  • Standard first year: $3,945 to $4,690 (application plus the first annual fee).
  • First year with expedited review: $6,445 to $7,190.
  • Every year after: $2,550 to $3,095.
  • Each additional facility: the same fees again, because both are charged per facility.

Smaller providers feel this most. In a Reddit social work thread about certification costs for small providers, a solo clinician described roughly $2,000 up front and about $1,000 a year as a wall they couldn't justify.

Then there's the cost that isn't on the fee page. LegitScript's standards include not being affiliated with lead generators. If your center buys leads, pays a referral service or lists with a call center that routes patients, that has to end or be cleared up before you apply. For some centers that's a bigger change than the fee. It also matters legally: buying addiction leads carries its own risk, which I cover in the answer on EKRA addiction treatment leads.

What certification unlocks, and what it doesn't

Google's healthcare and medicines ads policy requires LegitScript certification to advertise addiction treatment in the US and Canada. In Australia, France, Ireland and New Zealand, only government entities can run these ads at all. The policy also puts impulse-control, behavioral and nicotine addiction outside the certification, so a gambling or gaming addiction program can't use it to advertise those services. Meta has its own gate. Meta's policy on addiction treatment ads requires LegitScript certification and prior written permission from Meta. Certification is step one there, not the finish line.

And certification doesn't guarantee impressions. Centers report being certified, getting ads marked "approved (limited)" and then seeing almost nothing served. If that's happening to you, I go through why LegitScript ads are not showing in a separate answer.

It also doesn't protect your name. The FTC sued marketers for ads that used real treatment centers' names and routed the calls to their own call center. Watch your brand searches whether you advertise or not.

How to decide whether it pays back

I'd work through these before paying the nonrefundable application fee.

  1. Confirm your program is in scope. Read LegitScript's standards and Google's policy together. An owner in a Reddit Google Ads thread about a center ineligible for LegitScript described a program with no doctors and no medication-based treatment being unable to qualify, which closed paid search entirely.
  2. Clear out any lead-generator arrangements. List every paid referral source, directory and call service. End or document anything that could count as an affiliation before you apply.
  3. Get your paperwork and website in order. Licenses current, claims on the website accurate, staff and services described as they are. It's cheaper to fix these before review than after a rejection.
  4. Work out what one admission is worth to you. Use revenue after the direct cost of care, not the headline price. This is the number everything else is measured against.
  5. Divide the first-year cost by that number. That's how many extra admissions you need just to cover certification, before any ad spend. If one admission covers it, the decision is easy. Then set a separate ad budget with a maximum cost per admission you won't go past.
  6. Only pay to expedite if timing really matters. The extra $2,500 starts the review within two business days. It doesn't shorten anything after that.
Six steps before paying for LegitScript: confirm the program is in scope, clear lead-generator ties, fix paperwork and website, value one admission, divide first-year cost by it, expedite only if needed.
The application fee is nonrefundable, so the checks come before the payment.

Here's how step 5 plays out with the published fees. Say your first year costs $4,690 at the top of the range. If an admission leaves your center more than that after the direct cost of care, a single extra admission in the year covers certification, and everything after that is about whether the ads themselves pay. If an admission leaves you $1,500, you need four extra admissions just to break even on the certificate, and that's before a single click is paid for. Put your own figure in and the answer is usually obvious.

Do the same sum for year two, using only the annual fee. Some centers find year one is marginal but the ongoing cost is easy to justify once campaigns are running.

What's different for therapy practices

Therapists and counseling practices run into this without meaning to. In a Reddit PPC thread about ads restricted over addiction services, a practice that wasn't a rehab had its ads restricted because its website mentioned addiction services. For a practice where addiction work is a small part of the caseload, the practitioner route ($535 to apply, $1,070 a year) may be worth it, or it may make more sense to advertise the rest of your services and win addiction searches organically. Look at how much of your work it actually is before deciding.

If you're a group practice, remember the fees are per facility and per practitioner as applicable, so check which route covers your setup before you apply.

Questions treatment centers ask about LegitScript

Do I pay again for a second location?

Yes. Both the application fee and the annual fee are charged per facility, so a second site roughly doubles the cost. Budget each location separately and decide whether each one needs paid search.

Does LegitScript certification help my Google rankings?

I've seen no evidence that it does. It's a gate for advertising, not a ranking signal. Your organic visibility still depends on your website, your Google Business Profile and your reviews.

Can I advertise while my application is under review?

Not for addiction treatment in the US or Canada. The policy requires certification first, so plan the timing: apply well before you need the campaigns running, and keep organic work going in the meantime.

What not to do

  • Don't try to advertise around the requirement by rewording ads or landing pages to avoid the word addiction. If the service is addiction treatment, the policy applies.
  • Don't pay the application fee before checking eligibility. It's nonrefundable.
  • Don't buy leads while you're certified or applying. It conflicts with LegitScript's standards and carries legal risk.
  • Don't assume certification means traffic. Budget time to fix limited serving after approval.
  • Don't put all your growth into ads. Every rule above can change. Organic search and your Google Business Profile don't need certification.
